Practice at Leeds General Infirmary

MY PRACTICE AT LEEDS GENERAL INFIRMARY, LEEDS

My practice is exclusively in the field of trauma & limb reconstruction in the largest teaching hospital in Europe. The catchment area for trauma admissions from Leeds metropolitan area is approximately 850 000, while the hospital accepts tertiary referrals for complex fractures, open fractures, pelvis and acetabular fractures and spinal injuries from a region of 3 million people. My trauma practice involves not only the management of straightforward fractures encountered in any busy fracture clinic, but also the treatment of tertiary referral severe lower limb fractures, often treated in collaboration with our plastic surgeons. My elective orthopaedic practice comprises the investigation, diagnosis and treatment of nonunion, malunion and post-traumatic bone infection. Thanks to extensive Ilizarov training by the Russians, 14 months of training in limb reconstruction at Bristol Royal Infirmary, and almost four years in independent reconstructive practice, I am a national expert in trauma and limb reconstruction. My post was the replacement appointment for Malcolm Smith.

Since my appointment in July 2002 I have built up from scratch a limb reconstruction practice which comprises an all day specialist Ilizarov clinic; 2 experienced outpatient nurses to assist with pinsite care, pre-discharge ward visits and practical and psychological support for patients; an interested superintendent physiotherapist; and a weekly frame physiotherapy class. I was personally responsible for the introduction of a trust-wide evidence based pinsite care policy. This Russian-style pinsite care regimen has replaced the old British Consensus method, leading to a huge reduction in pinsite infections in Leeds. The significant reduction in pinsite complications has been audited and presented at the British Limb Reconstruction Society, European Trauma Congress, and British Orthopaedic Association meetings in 2004. This work on pinsite care is due to be published in the April 2006 British edition of the Journal of Bone & Joint Surgery.

I initiated the fostering of links with the limb reconstruction unit in Sheffield. We now hold a Leeds-Sheffield combined clinic every 3 months. Such collaborative working has proved invaluable in terms of personal and regional audit, clinical governance, research, and decision-making for complex reconstructive problems.

My practice at Leeds General Infirmary also includes the initial reception and stabilisation of pelvis and acetabular fractures; many aspects of management of children’s fractures; the initial assessment and emergency non-operative management of spinal trauma; and many aspects of management of upper limb fractures. In addition to expertise in the treatment of severe lower limb fractures and open fractures, I also have an interest in the treatment of displaced calcaneal fractures. I completed the AO calcaneal fracture course in 1999. I gained experience while working as specialist registrar for several national experts in the management of calcaneal fractures (Roger Atkins, Chris Curwen, Mark Jackson and Simon Gregg-Smith). I presented the Bristol experience of open calcaneal fractures at the British Orthopaedic Association in 1999. I treat many fractures using standard AO internal and external fixation techniques. I have completed Basic and Advanced AO courses. I was appointed an AO Instructor in 2003. I was appointed an Advanced Trauma Life Support Instructor in 2004. Following my appointment I initiated the transfer of all our implant trauma at Leeds General Infirmary to a clean air laminar flow theatre, leading to a significant reduction in deep sepsis rates for implant trauma.

Fellowship

As complex fracture management is my main interest, I have successfully undertaken clinical training in the application of the Ilizarov Method under the supervision of Professor Vladimir Shevtsov in Kurgan, Russia. The two month programme included an overview of Ilizarov methods; anatomy for pin placement; preoperative planning; bone model workshops and tips for stable frame construction; supervised surgery sessions; leg lengthening and bone transport techniques; post-operative management of wound sites and complications; physiotherapy and rehabilitation; and opportunity for emergency on-call for trauma. This fellowship followed on from an initial week’s course in Ilizarov application for foot, ankle and distal tibial pathology; including lectures, discussions and theatre sessions in foot and ankle fractures, pilon fractures, foot deformity correction, calcaneal fractures and post-trauma conditions.

I returned to the Ilizarov Centre for a further period of private study in the Department of Closed Trauma 1, dovetailing with a conference on the use of the Ilizarov Method in trauma treatment in 2002.

A third visit to Kurgan was undertaken to attend a conference on the use of the Ilizarov Method in the treatment of osteomyelitis in 2003.

In October 2004 I attended a conference on the basic science of distraction histiogenesis and spent further time in the Department of Bone Infection, working under the tuition of Head of the Department, Dr. Nikolai Klyushin.

In June 2006 I was invited to the Conference of Young Scientists at the Ilizarov Centre to celebrate the 85th anniversary of Professor Ilizarov’s life, and invited to make to presentations on the role of crusts in Ilizarov fixator pinsite care; and Flap & Frame, the combined orthoplastic management of adult open tibial fractures by microvascular free tissue transfer and fine wire fracture fixation.

My time in Russia studying the applications of the Ilizarov Method now totals four months.

Other Experience

Following Army sponsorship in 1987, I served with the Brigade of Gurkhas at British Military Hospital Dharan, Nepal, summer 1988. I gained experience in trauma and disaster management following the Nepal Earthquake, 21 August 1988.

In the aftermath of a large earthquake centred only 2 miles from the hospital, over 1000 casualties were treated by the hospital staff, with 500 arriving within the first 24 hours. The medical staff of the hospital consisted of a surgeon, an anaesthetist, a physician, two locally trained General Practitioners and three UK clinical medical students on elective. As a medical student, my role was to assess casualties at the receiving station; initiate basic life-supporting measures such as intravenous infusion and chest drain insertion; ensure adequate analgesia; and triage patients for emergency theatre, for inpatient wait for surgery or considered beyond help. In the ensuing days I was personally responsible for the ward care of many of the survivors.

Audit

I regularly attend and contribute to the monthly departmental audit meetings at St. James’s University Hospital, Leeds.

My work auditing pinsite infection rates using Russian-style pinsite care versus British consensus method was published in the British edition of the Journal of Bone and Joint Surgery in Spring 2006. Further audit work comparing pinsite infection rates in Russian-style pinsite care with crusts removed or retained has recently been submitted to the British Journal of Bone and Joint Surgery.

Teaching

I am strongly committed to teaching and have attended the Royal College of Surgeons Training the Trainers course, the Stratec AO Instructors course, and the ATLS Instructors Course.

I am an AO Instructor.

I am an ATLS Instructor.

In collaboration with my colleague Toby Branfoot I help to organise and teach on the Leeds-Sheffield Basic External Fixation Course.

I teach on both basic and advanced Ilizarov courses run in Leeds, Sheffield, Liverpool and Edinburgh.

I examine on the Bristol FRCS (Tr & Orth) intensive revision course.

I have been actively involved in teaching medical students since my teaching hospital house job at Southampton General Hospital, and I continued this through my SHO posts and during my time as Specialist Registrar. While a registrar at Royal United Hospital, Bath I organised the orthopaedic revision course for final year medical students from Southampton University Medical School. More recently I was involved in the teaching of clinical medical students from Bristol University in their musculoskeletal systems course at Bristol Royal Infirmary and Frenchay Hospital, Bristol. While at Bristol Royal Infirmary I taught on the ENB 219 orthopaedic nursing course at the University of the West of England and I have also been involved as lecturer and table demonstrator on the AO study day for theatre nurses.

I regularly teach on the Children’s Surgical Nursing course at St. James’s University Hospital.

While at St. James’s I have helped to set up the multidisciplinary Foundation Course in Trauma and Orthopaedics, which is aimed at qualified staff nurses on the orthopaedic wards and senior house officers in trauma and orthopaedics.

I annually teach at the Sheffield Trauma Summer School on aspects of orthopaedic trauma management, and was recently invited to Utrecht, Netherlands to talk to the Dutch orthopaedic trainees on the treatment of complex tibial plateau fractures using the Ilizarov Method.

Management Experience

Apr 1999 – Jun 2000 “Who Cares?” management training programme

I participated in the “Who Cares?” management training programme organised by NHS South & West Management Executive. This was a fourteen month day release course meeting formal training requirements in management and management development. It was held jointly with management and finance trainees from the NHS Management Training Scheme. Core topics included the history and structure of the NHS; statutory frameworks for the NHS and employment legislation. Team building and leadership included outdoor events, psychometric testing and personality types, stresses within teams and building high performance teams. Other core topics included new advances in health technology; the role of management; and the role of healthcare professionals including clinical governance, evidence-based medicine, Calman, NICE and appraisal / revalidation. It also involved finance and funding structures; strategic and operational planning; organisational power frameworks; the role of patient groups; and the role of arbitration, mediation and conflict management.

I currently chair the Chancellor Wing theatre users’ group monthly multidisciplinary meetings at St. James’s University Hospital, Leeds. I am responsible for supervision of the Basic Surgical Trainees rotating through the department. I am involved in interviewing for Pre-Registration House Officers, Senior House Officers, and SSHO posts within the West Yorkshire Deanery, in addition to annual involvement in interviewing candidates for Leeds University Medical School.

Other Interests

SPORTS – I represented my school, medical school and the Royal United Hospital XV at scrum half. I enjoyed watching Bath and Bristol play rugby at home, but more recently I have switched codes to support Leeds Rhinos rugby league team.

I enjoy walking in the Lake District and Peak District.

MODERN LANGUAGES – I speak French. I have been learning Russian at nightclass, and I was recently awarded the Foundation Qualification in Modern Languages (Russian) from Leeds Metropolitan University.

TRAVEL – I enjoy foreign travel, particularly in Europe. I have visited France, Luxembourg, Cyprus, Switzerland, Germany, Republic of Ireland, Denmark, Austria, Holland, Greece, Russia, Finland, Spain, Czech Republic, Italy, Sweden, Estonia, Portugal, Belarus, Slovakia and Belgium. I have spent time in Asia including Nepal, Siberia, Borneo, China and Mongolia. I have undertaken several trips to the USA, including New Jersey, Philadelphia, New York, Washington DC and Tennessee. I have made one visit to Australia. I also like travel within the British Isles, particularly the Lake District and South West England.
